I was going through the extant version, playing the melody on my guitar, and noticed that it had some missing bars and no third ending (it tries to repeat the first ending for third, but if you do that, you expunge four bars).

So here's a corrected chart utilizing the changes from Sher Music's version (pretty close to existing version):
