A friend was looking for a chart of this—I finally ran it to ground in the New Real Book 3.
It took some finagling to get it to play in iReal Pro the same way as the chart reads, but here you go…

NRB3's letters A & B are played as part of the Intro (Head In)
Solos written as ABC on the chart becomes just letter A in iReal Pro
Last A & B with jump to Coda become Coda (Head Out)
Last chord can be resolved to Eb MA9 if you want, but I like it hanging …
