Bob, I couldn't find what you were talking about in the coda, but I did find that the C#m bar right before the 1st ending had only four spaces instead of five, so I fixed that.

Ending Volta Bracket Placement - Example

Don't worry about changing the number of spaces in each measure as you change time signatures. It's not neccessary and usually doesn't matter.

Ending indicators are correctly entered in the first space of the first bar of the ending. Placing them anywhere else is unusual.
Can you see the difference in the example chart I posted for you?
